Registration and Equipment Order Form - 2021/2022

This is the order form for clothing and equipment for the 2021/2022 school year. Note that some items are required and others are optional. A completed order form will be emailed to the email address entered below. Print this form and submit it along wth your check. Please write the name of your student on the check. Complete a separate form for each student.

If you do not receive your confirmation email, check your spam folder. If it's not there contact the site admin (there is a link under Contacts on this site).


Note: a copy of this form, which you need to turn in with your check, is sent to this email address.

  • Select "Replacement Order" if you already filled out this form online but wish to replace that form with this new one.
  • Select "Add-On Order" if you are ordering materials in addition to the order form already submitted.


There is a $100 charge per student to cover costs including band camp and services throughout the year such as pizza and pop at games, water at parades, and icy pops at camp.


Parents/guardians, siblings, etc. may also order. Include sizes

Tux Shirt/Bow Tie

Used by Symphonic Band 10, Symphonic Band 11/12 and Jazz Band. Note: Shirts purchased last year will carry over to this year. If you are not sure, place an order, and we will reimburse if necessary.

"W" are misses sizing. If you are unsure on men's sizing go up to the next size.

Dress Tux Shirt Size Chart

The tux shirt order comes with a bow tie. Order a bow tie if you want an extra one or your previous tie was lost or damaged.

Please write one check per family payable to NHS Band (no tax). Please put the printed form (it will be emailed to you) and payment in an envelope (no staples, please) with student's name and "Band Registration" or "Add-on Order" and place in white mailbox in the band room.

All payments due Monday, August 16th

Show shirts can be ordered up to September 3rd

Please contact Mrs. Ray-Hepp if you need to make an alternate payment schedule. No one is excluded from band based on ability to pay.


Please select one or more areas where you have an interest in volunteering. Someone will contact you about specifc volunteer opportunites shortly. (Note - you must select at least one to submit the order)
