Registration and Equipment Order Form - 2019 / 2020

This is the order form for clothing and equipment for the 2019 / 2020 school year. Note that some items are required and others are optional. A completed order form will be emailed to the email address entered below. Print this form and submit it along wth your check. Please write the name of your student on the check. Complete a separate form for each student.

If you do not receive your confirmation email, check your spam folder. If it's not there contact the site admin (there is a link under Contacts on this site).


 

Note: a copy of this form, which you need to turn in with your check, is sent to this email address.

  • Select "Add-On Order" if you are ordering materials in addition to the order form already submitted.

 

There is a $100 charge per student to cover costs including band camp and services throughout the year such as pizza and pop at games, water at parades, icy pops at camp, a picnic, and an ice cream social.

Sizing determined on fitting day, Wed. Aug. 15th at 1 pm. (NOTE: Attending the sizing is required to get shirt ordered)

Please contact Mrs. Ray-Hepp if you need to make an alternate payment schedule. No one is excluded from band based on ability to pay.
